What is the job role?

We require a Social Worker to join a dedicated Multi-Professional Team to help assess and arrange discharge plans for people in the hospital either informally or under the Mental Health Act. The role will cover people in hospitals across Coventry and Warwickshire including The Caludon Centre, St Michaels Hospital in Warwick, and The Avenue Clinic in Nuneaton.

You will be involved in arranging and assessing people’s needs under The Care Act 2014, aftercare under the Mental Health Act 1983 (revised 2007), and the Mental Capacity Act 2005. 

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

 

Who are we looking for?

We need a qualified Social Worker who has:

  • Excellent assessment skills
  • Working knowledge of and application of  MHA, MCA, The Care Act, and Children’s Act
  • Organisation, recording, and workload management skills
  • Diplomacy, negotiating, and listening skills.
  • Be able to work in partnership with users, carers, and colleagues.
  • To work as part of a team
  • Have autonomy and decision-making skills
  • Develop personal home treatment plans
  • Good IT skills
  • Have a vision for your own career development and future training
  • Resilience, flexible, and have stamina
