What is the job role?
We have a Community Care Worker post within our Community Discharge team. You will be supporting the safe and timely discharge of people in hospital who have a wide range of social care issues. There is a real emphasis on promoting independence.
The service will operate across 7 days, Monday – Friday 8.00am – 8.00 pm and Weekends 8.00am – 4.00 pm for which a shift allowance of 10% is paid. We have two positions; one permanent position and one temporary maternity leave cover

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).
All candidates must have the Right to Work in the UK. We are currently not offering sponsorship for this role.
Who are we looking for?
We are looking for people with a passion for promoting independence and making the principles of the Care Act 2014, a reality for the people of Coventry.
If you like to work as part of a team but can also use initiative and work autonomously, we would really like to hear from you.
As the successful candidate you will have:
- Experience of assessment and case management work with older people, people with physical disability, people with learning disability or other adult service users.
- Ideally have experience of recording and record keeping procedures.
- Experience of team membership and participation.
- Experience of working with a range of service user groups either in a personal or professional capacity.
- Have very good organisational skills.
