What is the job role?

As a Mental Health Community Case Worker, you will play a key role in supporting adults to live as independently and safely as possible. You’ll be responsible for receiving referrals, undertaking assessments, and developing outcome-focused care plans that promote choice, control, and wellbeing. Working within legal frameworks and departmental policies, you will manage a varied caseload, maintain accurate records, and coordinate cost-effective care packages tailored to individual needs. 

 The role also involves identifying and responding to safeguarding concerns, contributing to legal processes where required, and arranging and leading case reviews and planning meetings. You will be supported by experienced managers and work closely with qualified Social Workers when more complex intervention is needed.

 We have two roles:  One role will be located in the Crisis Resolution and Home Treatment Team and the other in Dementia and Older Peoples Team in Longford.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

 All candidates must have the Right to Work in the UK. We are currently not offering sponsorship for this role.

Who are we looking for?

We’re looking for someone who is organised, proactive, and committed to delivering high-quality support to vulnerable adults. You’ll have strong communication and assessment skills, with the ability to manage a busy caseload and make informed decisions in line with policy and legislation.

 You should be confident working both independently and as part of a multi-agency team, with a good understanding of safeguarding, risk management, and person-centred care. A willingness to learn and stay up to date with relevant legislation, services, and best practice is essential. Most importantly, you’ll be passionate about helping people maintain their independence, dignity, and quality of life. 

 If you are ready for the challenge, you will need to demonstrate:

 • You practice with authenticity, commitment and respect at all times

• You are an excellent team player

• You have a working knowledge and understanding of the common social issues that impact an individual experiencing acute and/or enduring mental disorder, and the range of interventions that can be made available to address those social needs

• You have an understanding of assessing risk and how to escalate to relevant professionals 

• You can explore, identify and implement creative social solutions to prevent/reduce the need for a psychiatric inpatient admission, including knowledge of the creative use of direct payments

• You can identify and effectively mobilise relevant agencies, professionals and resources that are central to an individuals’ recovery plan,

•  You have the ability to form robust and purposeful relationships with individuals, families, carers, and the partner agencies you work with.
