This is an exciting opportunity to join our new  Peripatetic Team, a flexible team that provides support across Adult Social Care services when and where there is increased demand. You will work in different teams and settings for agreed periods, supporting to make sure people continue to receive safe and high quality care during times of increased need.

What is the job role?

We are recruiting two Community Case Workers to join our Adult Social Care team in a peripatetic role. This is a great opportunity for motivated individuals who enjoy working across a variety of settings and delivering person-centred support.

You will assess the needs of service users and carers, develop and review outcome-focused care plans, and promote independence. Working closely with colleagues and partner agencies, you’ll help ensure services are safe, effective, and meet agreed needs. The role also involves risk assessment, managing challenging situations, and maintaining accurate digital records.

All candidates must have the Right to Work in the UK. We are currently not offering sponsorship for this role.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

Who are we looking for?

We are looking for organised and adaptable individuals with experience in assessment and case management across a range of adult service user groups.

You will have:

  • Strong communication and relationship-building skills 
  • Experience developing and reviewing care plans 
  • Confidence in risk assessment and managing complex situations 
  • Good ICT and record-keeping skills 
  • The ability to prioritise work and meet deadlines
