About the team we are recruiting to

Adult Social Care is part of the People Directorate within Coventry City Council.  We have a simple vision and strategy that defines what we are trying to achieve, our purpose and our approach which underpin the principles of Adult Social Care, and we continue to work in support of this.

In a simple sense all our work, at whatever level should continue to support the strategy of: ‘Providing support, in the least intrusive manner possible, based on the assets, resources and abilities that are available to people’.

Adult Social Care encompasses a wide range of vital services covering;

  • Commissioning Team – we commission and work with a number of providers, to ensure that people with care needs have access to a range of good quality support to enable them to live independent and fulfilling lives
  • Social Work Services - we work with individuals who have care and support needs, and with their carers be they older or younger adults
  • Occupational Therapy and Equipment Services - we aim to enhance people’s ability to live safely and independently in their home
  • Internally Provided Services – we provide direct support to people enabling those in most need to live independent and fulfilled lives
  • Business Systems and Continuous Improvement - we provide a range of services including Financial Assessments, provider payment processing and Business Systems administration. The Business Systems team provide support to Adult Social Care, Children’s Services and Education Services.

What is the job role?

Come and join the All Age Disability Service as a Team Leader for our Transitions Team. The Adult Disability Service is looking to recruit a Team leader for the Transitions Team on a permanent basis. This is an exciting opportunity to provide high quality transitions from children's services. This is an established and experienced team.

The role requires skills and knowledge regarding managing social care practitioners who complete Care Act assessments and safeguarding adults activity in relation to adults with a physical or learning disability, awareness of transforming care, and promoting independence agendas and supporting planned transitions for young people.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

 

Who are we looking for?

We are looking for a dynamic, organised and improvement focussed manager. If you enjoy a challenge and welcome the autonomy to work creatively and to demonstrate best practice, then this is the post for you.

A young person’s journey to adulthood is a crucial time in their development. The transitions team plays a crucial role, working in partnership with children’s services, education and other agencies to enable young people to prepare for adulthood in the best possible way.

You will need to hold a Social Work Qualification and be registered with SWE (Social Work England) and to have some supervisory experience is preferable. You will have excellent knowledge and practice experience of working with people who have complex needs, and require integrated support working alongside health colleagues.

You would need to be able to work both autonomously and in partnership with health services, partner organisations, to manage a service which assesses individuals adults with a disability, and work to  promoting  independence.  You will need to demonstrate a high standard of leadership, performance management and commitment to providing a high quality service.

You will be supported by a Service Manager for All Age Disability service and will manage an experienced and highly skilled team devoted to achieving the best outcomes for people and their carers.
