What is the job role?
We have an exciting opportunity for someone looking to join a dynamic motivated group of professionals. The role is ideal for someone already in a team leader post or a social worker looking to develop their career.
This role is to work within the transitions part of the All Age Disability team, this will involve supporting the social work team to support and plan for young people who are approaching adulthood, or leaving education. This role will involve working closely with our partner agencies including education, children's services and health. The role sits within the wider All Age Disability Team, therefore you would also be required to support the wider team when required.
The All Age Disability Team, work with adults, with a range of disabilities and or complex health needs. Our focus is on maintaining independence and using a strengths based model to enable people living in Coventry to maximise their outcomes.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).
Who are we looking for?
You will be expected to carry out professional supervision and support the development of your team and their continuous professional development.
The role requires you to lead on safeguarding enquiries, act on behalf of the service manager in their absence, manage performance, oversee the allocation process, monitor and improve quality, and contribute to the overall development of the service.
This is a challenging but fulfilling role, within a busy vibrant team. We are looking for a qualified Social Worker who is registered with Social Work England, who has experience working within Adult Social Care.
The ideal candidate will have a good working knowledge of the Care Act 2014 and the Mental Capacity Act 2005, SEND Code of Conduct and be able to support their team by working within this legislation. We are looking for an individual who is committed to improving the lives of young adults with disabilities, and who understands the impact of the inequalities faced by people with disabilities.
