What is the job role?
We have an exciting opportunity for someone looking to join a dynamic motivated group of professionals. The role is ideal for someone already in a Team Leader post or a social worker looking to develop their career.
The Community Discharge Team Hospital, works with adults, with a range of disabilities and or complex health needs. Our main focus is to provide a timely, supported discharge service to people in hospitals who have a wide range of Social care issues. maintaining independence and maximising the use of short-term services to ensure people can access enablement to reach their outcomes and potential following hospital admission.
The service operates across 7 days, Monday – Friday 8.00 am – 8.00 pm and Weekends 8.00 am – 4.00 pm for which a shift allowance of 10% is paid.
You will be expected to carry out professional supervision and support the development of your team and their continuous professional development.
The role requires you to lead on safeguarding enquiries, act on behalf of the service manager in their absence, manage performance, oversee the allocation process, monitor and improve quality, and contribute to the overall development of the service.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).
Who are we looking for?
This is a challenging but fulfilling role, within a busy vibrant team. We are looking for a qualified Social Worker who is registered with Social Work England, who has experience working within Adult Social Care. We are looking for people with a passion for promoting independence and making the principles of the Care Act 2014, a reality for the people of Coventry.
The ideal candidate will have a good working knowledge of the Care Act 2014 and the Mental Capacity Act 2005, and be able to support their team by working within this legislation.
