What is the job role?

This is an excellent opportunity to join our Mental Health Services as a Community Case Worker.

You will work closely and cohesively with NHS, Social Care, and Third Sector colleagues to assess, identify and arrange interventions based on social and recovery models.

You will have a prominent role in supporting individuals who require a more intense, personalised, and specialised response from Social Care.

You will be located initially within The Mental Health and Wellbeing Team at Tile Hill Health Centre and will work closely as part of a small team of experienced social care practitioners from THE POD and Axholme Services to deliver and develop its interface with the Community Teams.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

  • Excellent assessment skills
  • Working knowledge of, and application of  MHA, MCA, The Care Act, and the Children’s Act
  • Organisation, recording, and workload management skills
  • Diplomacy, negotiating, and listening skills
  • Be able to work in partnership with users, carers, and colleagues
  • To work as part of a team
  • Have autonomy and decision-making skills
  • Help to design and deliver  personal home treatment plans
  • Good IT skills
  • Have a vision for your own career development and future training
