About the team we are recruiting to

This is an exciting opportunity to work in the Coventry Community Mental Health Team. You will be working alongside Coventry and Warwickshire Partnership Trust colleagues to provide a seamless service to citizens with severe and enduring mental health challenges. The team is a multidisciplinary service, and we work closely with nurses, occupational therapists, psychologists and psychiatrists to deliver person centred care to our citizens.

What is the job role?

The Community Support Worker is an integral part of the social care service which is delivered as a member of a multi-disciplinary team for people with mental health needs, their family groups or carers, within the community and in-patient mental health facilities.

Main Duties and Responsibilities:

Included but not limited to:

  • Undertake assessments under the Care Act 2014 to establish and identify unmet social care needs with a focus on strengths and recovery focused. 
  • Implement care and support plans for citizens and carers which are proportionate, and person centred. Be involved in reviewing and evaluating to effectiveness of care plans and services.
  • Be a proactive member of the team, offering support to colleagues through attending daily handovers and offering ad hoc support at times of crisis for citizens.
  • Attend multi-disciplinary meetings whether within the team or external teams i.e. inpatients.
  • Maintain effective liaison with statutory, voluntary and independent sector agencies on behalf of service users and to assess and co-ordinate a range of specific services in respect of care management to meet their needs.
  • Attend and participate in regular Team Meetings.
  • Foster professional development by participating in training programmes, courses and seminars in consultation with and the approval of the Team Manager.
  • Participate in regular supervision.
  • Maintain prompt accurate records and other documentation relating to work with service users in accordance with approved policy and procedures.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

Who are we looking for?

We would welcome and encourage applicants who are experts by experience.

If you are ready for the challenge you will need to demonstrate:

  • you practice with authenticity, commitment and respect at all times
  • you are an excellent team player
  • you have a working knowledge and understanding of the common social issues that impact on an individual experiencing acute and/or enduring mental disorder, and the range of interventions that can be made available to address those social needs
  • you have an understanding of assessing risk and how to escalate this to relevant professionals and services
  • you have the ability to explore, identify and implement creative social solutions to prevent the need for a psychiatric inpatient admission, including knowledge of the creative use of direct payments
  • you have the ability to identify and effectively mobilise relevant agencies, professionals and resources that are central to an individuals’ recovery plan,
  • you have the ability to form robust and purposeful relationships with individuals, families, carers, and the partner agencies you work with.
  • You have strong ICT skills
