About the team we are recruiting to

Adult Social Care is part of the People Directorate within Coventry City Council.  We have a simple vision and strategy that defines what we are trying to achieve, our purpose and our approach which underpin the principles of Adult Social Care, and we continue to work in support of this.

In a simple sense all our work, at whatever level should continue to support the strategy of: ‘Providing support, in the least intrusive manner possible, based on the assets, resources and abilities that are available to people’.

Adult Social Care encompasses a wide range of vital services covering;

  • Commissioning Team – we commission and work with a number of providers, to ensure that people with care needs have access to a range of good quality support to enable them to live independent and fulfilling lives
  • Social Work Services - we work with individuals who have care and support needs, and with their carers be they older or younger adults
  • Occupational Therapy and Equipment Services - we aim to enhance people’s ability to live safely and independently in their home
  • Internally Provided Services – we provide direct support to people enabling those in most need to live independent and fulfilled lives
  • Business Systems and Continuous Improvement - we provide a range of services including Financial Assessments, provider payment processing and Business Systems administration. The Business Systems team provide support to Adult Social Care, Children’s Services and Education Services.

What is the job role?

A national drive to reshape Adult Mental Health Services has led to Coventry being awarded NHS England funds to deliver a new, innovative, inter-disciplinary social interventions collective (SICol). The vision and passion of SICol are premised on delivering a human response to a human situation through strengths-based assessment and intervention to individuals experiencing acute and/or enduring mental disorders. It aims to offer a response that inspires hope and recovery through co-producing solutions to complex situations which cannot be addressed by medical interventions alone.

You will work closely and cohesively with NHS, Social Care, and Third Sector colleagues to assess, identify and arrange interventions based on social and recovery models. You will have a prominent role in supporting individuals who require a more intense, personalized and/or specialized response from SICol and will be required to present cases to the SICol MDT and other relevant MDTs.

You will be located within the Caludon Centre and will work closely as part of a small of team of experienced social care practitioners from THE POD and Axholme Services to deliver and develop its interface with the Community Teams.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

 

Who are we looking for?

  • you practice with authenticity, commitment and respect at all times
  • you have a working knowledge and understanding of the common social issues that impact on an individual experiencing acute and/or enduring mental disorder, and the range of interventions that can be made available to address those needs
  • your skills, abilities and knowledge to assess and arrange personalised care and support, including mobilising the creative use of direct payments
  • your ability to identify and effectively mobilise relevant agencies, professionals and resources that are central to an individuals’ recovery plan
  • your ability to form robust and purposeful relationships with individuals, families, carers, and partner agencies you work with.
  • your ability to explore, identify and implement creative social solutions to prevent/reduce the need for a psychiatric inpatient admission

You must have:

  • A clear enhanced DBS check
  • Substantial experience of working with individuals or have lived experience of severe and enduring mental illness
